(9) The order of dissolution shall include an affidavit from the Respondent stating that she is not the child's natural parent of the Respondent and stating that the Respondent will not be found by a court of competent jurisdiction to be the child's natural father, and of which the Respondent shall be notified at that time.
(10) If the marriage of the Petitioner and the Respondent is not permanently dissolved, the consent of the Petitioner will continue to be required to give effect to this proceeding, and the Respondent shall be responsible for compliance therewith. The parties shall be responsible for the payment of any costs of this proceeding.
